Notice Of Privacy Statement

Crescent Court is required to track disclosures of protected health information. The purpose of tracking disclosures is to provide an individual with an account of disclosures for six years prior to their request (from the date the document was created).
Please be advised that tracking is not required for treatment, payment, or for the healthcare operations.

Crescent Court will provide residents with access to their Protected Health Information (with limited exceptions) contained in their Clinical Record maintained by this facility.

Crescent Court shall obtain a resident's (or responsible party) written authorization prior to utilizing or disclosing a resident's Protected Health Information including photographs/ video for purposes of marketing. Crescent Court will follow the minimal necessary rule allowing for use or disclosure of Protected Health Information, which is minimally necessary to accomplish the purpose of use for the disclosure.

Crescent Court will provide residents with the opportunity to request a restriction on the use and/or disclosing of their Protected Health Information. Resident / Responsible Party have the right to request amendment of their Protected Health Information.
